This project will test whether adding jump-rope exercises to play activities helps improve foot posture in 4–15-year-old children with Down syndrome who have navicular drop.

What this trial studies

This is a randomized controlled trial enrolling 32 children with Down syndrome and navicular drop at sites in Faisalabad and Lahore. Participants will be randomly assigned to an experimental group doing jump rope plus play activities or a control group doing play activities alone, with measurements taken before and after the intervention. Foot posture will be measured using the Navicular Drop Test and the Foot Posture Index, and data will be analyzed using SPSS. Children with recent lower-limb surgery, significant sensory impairments, trauma, or inability to participate will be excluded.

Who should consider this trial

Good fit: Ideal candidates are children aged 4 to 15 years with Down syndrome who have navicular drop/prone foot and who can safely participate in supervised jump-rope and play activities without recent lower-limb surgery or trauma.

Not a fit: Children with visual or auditory impairment, recent lower-limb surgery or trauma, significant behavioral noncompliance, or inability to take part in supervised exercise are unlikely to benefit from this program.

Why it matters

Potential benefit: If successful, the program could reduce navicular drop and improve foot posture and mobility, making walking and play easier for affected children.

How similar studies have performed: Exercise programs to strengthen foot muscles have shown some benefits in related populations, but using jump rope specifically for navicular drop in children with Down syndrome is relatively novel and direct evidence is limited.

Inclusion Criteria:

* age of 4 to 15 years
* navicular drop
* prone foot
* Acute musculoskeletal injuries
* Normal: 5-10 mm
* Excessive Drop: \>10 mm (indicating possible flatfoot)
* Minimal Drop: \<5 mm (indicating a higher arch)

Exclusion Criteria:

* visual or auditory impairment
* lower limb trauma
* recent Surgical Intervention on lower limb
* Non- compliance and behavioral issue
* Inability to participate
