What this trial studies
The ITACARE-P CardioRehab Registry is a nationwide, multicenter observational project coordinated by Fondazione Don Carlo Gnocchi in Milan that enrolls adults with coronary artery disease and/or heart failure who are eligible for cardiac rehabilitation. It gathers standardized clinical, functional, treatment, and program-delivery data from hospitals and rehabilitation centers offering inpatient and outpatient CR. The registry tracks outcomes such as exercise capacity, medication use, readmissions, and quality of life over time to identify which program features and patient subgroups are linked to better recovery. Collected data will be used to describe variation in referral and care pathways and to inform efforts to standardize and improve cardiac rehabilitation services in Italy.
Who should consider this trial
Good fit: Adults with coronary artery disease and/or heart failure who meet national criteria for referral to cardiac rehabilitation, whether entering inpatient or outpatient programs, are eligible to participate.
Not a fit: Patients who explicitly refuse participation or who expect a planned hospital readmission within six months after finishing cardiac rehabilitation are not expected to benefit from the registry's findings.
Why it matters
Potential benefit: If successful, the registry could help increase appropriate referrals, standardize rehabilitation programs, and improve recovery and reduce readmissions for patients with coronary disease or heart failure.

Inclusion Criteria: * Patients with coronary artery disease and/or heart failure who meet the current regulatory requirements for appropriateness of referral to cardiac rehabilitation are eligible for enrollment. These patients may be included regardless of whether they are referred to inpatient or outpatient cardiac rehabilitation programs, depending on the clinical setting and care pathway. Exclusion Criteria: * Explicit refusal to participate in the study by the patient; * Planned hospital readmission within six months after completion of the cardiac rehabilitation program.
