We will test an interactive digital breathing game and paired device to help children aged 7–18 with bronchiectasis do airway-clearing exercises at home without a physiotherapist.

What this trial studies

The project will develop a Turkey-made digital respiratory game (DRG) and a sensing device that detect lung volume and flow and provide real-time guidance for Active Cycle of Breathing Techniques (ACBT). Clinically stable children aged 7–18 will use the home-based system on a smartphone, tablet, or computer to perform daily airway-clearing maneuvers. The DRG includes difficulty levels, storytelling, and feedback to improve engagement and adherence while measuring technique and usage. Outcomes will include adherence, quality of breathing maneuvers, and respiratory measures collected during the intervention and follow-up.

Who should consider this trial

Good fit: Ideal candidates are children 7–18 years old with bronchiectasis who are clinically stable, have access to a smartphone/tablet/computer, and have no cognitive or major comorbid conditions that would prevent using the system.

Not a fit: Patients with recent supervised physiotherapy, history of lung or liver transplant, active exacerbations or infections, significant cognitive impairment, or concurrent cardiac/orthopedic/neurological disease are unlikely to receive benefit from this intervention.

Why it matters

Potential benefit: If successful, the tool could improve daily airway clearance and adherence, reduce respiratory infections and clinic visits, and let children manage therapy at home without frequent physiotherapist supervision.

How similar studies have performed: Related digital respiratory games and home airway-clearance devices have shown promise in small pilots—particularly in cystic fibrosis—for improving adherence and technique, but evidence specifically in pediatric bronchiectasis is limited and this Turkey-developed system is novel.

Eligibility criteria

Inclusion Criteria:

* Aged between 7-18 years
* Clinically stable
* No exacerbations or infections
* Possesses a computer, tablet, or mobile phone

Exclusion Criteria:

* Participation in a supervised physiotherapy and rehabilitation program within the last 6 months
* History of lung or liver transplant
* Presence of any cognitive impairment that would hinder the use of the system
* Changes in medical treatment during the study
* Presence of concurrent cardiac, orthopedic, or neurological diseases
