Physical Activity in Adolescents With Chronic Kidney Disease
This 12-week program will test if an online physical-activity plan can add about 15 minutes of moderate-to-vigorous activity per day for adolescents (ages 13–18) with chronic kidney disease.

What this trial studies
Cardiovascular disease is a leading cause of illness and early death in youth with chronic kidney disease, and these adolescents are generally less active than their peers. Investigators will enroll 64 adolescents with CKD into a 12-week physical activity intervention and use the Multiphase Optimization Strategy (MOST) framework to optimize which components work best. The optimization trial aims to find intervention elements that increase average daily moderate-to-vigorous physical activity by 15 minutes, a clinically meaningful change. Intervention components grounded in behavior-change theory will be delivered via the Way to Health web platform to reduce clinic time and resource barriers.
Who should consider this trial
Good fit: Adolescents aged 13–18 with CKD stage II–V who are not on dialysis or transplanted, speak English, have parental permission, and have baseline activity under 45 minutes/day are ideal candidates.
Not a fit: Those on dialysis, with a prior solid-organ transplant, who are non-ambulatory, pregnant or lactating, have very high baseline activity, limited English proficiency, or medical conditions that make activity unsafe are unlikely to benefit or be eligible.
Why it matters
Potential benefit: If successful, the program could increase daily activity, improve fitness and quality of life, and help lower future cardiovascular risk in adolescents with CKD.
How similar studies have performed: Physical-activity programs have improved activity in healthy youths, but optimized, web-delivered interventions specifically tested in adolescents with CKD are limited, so this approach is relatively novel in this population.

Inclusion criteria: For adolescent participants: 1. Males or females age 13 - 18 years 2. Diagnosis of CKD stage II - V not yet on dialysis or with a transplant 3. English proficiency 4. Parental/guardian permission (informed consent), and if applicable, child assent For caregiver participants: 1. Parent/legal guardian of an enrolled adolescent participant 2. English proficiency Exclusion Criteria: For adolescent participants: 1. Pregnant or lactating 2. Non-ambulatory 3. Have high physical activity at baseline (\>45 min/day of MVPA) 4. Provide \<2 days of physical activity data during the run-in period 5. Have a condition, such as heart or lung disease, that preclude safely engaging in physical activity 6. Have received a solid organ transplant 7. Have limited English proficiency 8. Are on dialysis 9. In the opinion of the investigator, are unable to comply with the requirements of the study protocol or are unsuitable for the study for any reason For caregivers participants: 1\. Limited English proficiency
