What this trial studies
This study aims to enhance the care of patients with Chronic Obstructive Pulmonary Disease (COPD) by implementing a clinical prediction model called ACCEPT into electronic health records at two teaching hospitals in Vancouver, Canada. The ACCEPT model predicts individual patient risks for exacerbations, allowing for personalized treatment plans. A stepped wedged cluster randomized controlled trial design will be used to evaluate the effectiveness of this approach over a 30-month period, assessing both patient outcomes and satisfaction. The goal is to determine if this model leads to better management of COPD and improved patient experiences.
Who should consider this trial
Good fit: Ideal candidates for this study are Canadian residents aged 35 years and older who have a diagnosis of COPD and can communicate in English.
Not a fit: Patients under 35 years of age or those without a diagnosis of COPD will not benefit from this study.

Inclusion Criteria: * Are a legal Canadian resident * Aged 35 years and older * Can speak English * Have a diagnosis of COPD Exclusion Criteria: • Are under 35 years of age
