What this trial studies

This study aims to assess the nursing care needs of elderly individuals living in the Évora district who have chronic diseases. It utilizes the 'Self-Care of Chronic Illness Inventory - Patient Version (version 4)' to identify self-care deficits among participants. The goal is to develop a continuous care model that enables elderly patients and their caregivers to manage their health at home, thereby preventing health crises that may require emergency care. The study focuses on enhancing the quality of life for elderly individuals by promoting effective self-management of their chronic conditions.

Who should consider this trial

Good fit: Ideal candidates for this study are individuals aged 65 or older who reside in the Évora district and have at least one chronic disease.

Not a fit: Patients who are younger than 65 or do not have any chronic diseases may not receive benefit from this study.

Why it matters

Potential benefit: If successful, this approach could significantly improve the health-related quality of life for elderly patients managing chronic diseases.
