What this trial studies
This study investigates the impact of training healthcare professionals on medication assessments for nursing home residents with limited life expectancy. It employs a cluster randomized controlled trial design, involving six nursing home organizations and aiming to include 450 patients aged 65 and older. The intervention focuses on tailoring medication use to align with the palliative treatment goals of these residents. The study will last for 15 months, starting in September.
Who should consider this trial
Good fit: Ideal candidates are nursing home patients aged 65 and older with a life expectancy of less than 1.5 to 2 years.
Not a fit: Patients with a life expectancy of less than 4 weeks or those expected to live significantly longer than 1.5 to 2 years may not benefit from this study.

Inclusion Criteria: Nursing home patients: * Age 65years and above * Living at long term care wards in nursing homes * Living at a long term care ward for at least 4 weeks before start of the study Exclusion Criteria: * Temporary nursing home patients (i.e. geriatric rehabilitation or end-of-life/hospice care) * Life expectancy \< 4 weeks * Life expectancy clearly \> 1.5 - 2 years (i.e. patients with Huntingtons disease)
