What this trial studies

This trial aims to enhance the identification and management of chronic kidney disease (CKD) among patients in Stockholm through an electronic clinical decision support (CDS) system. Conducted across 66 primary healthcare centers, the study will randomly assign these centers to either receive the CDS trigger or basic advice for 18 months. The CDS system is designed to assist general practitioners in following guideline-recommended processes for CKD care, addressing the significant under-detection and undertreatment of this condition. The trial will evaluate improvements in CKD care indicators among the participating centers.

Who should consider this trial

Good fit: Ideal candidates include adults with diabetes, hypertension, or existing chronic kidney disease, as well as those showing signs of kidney function decline.

Not a fit: Patients without any of the specified conditions or those who do not require annual kidney disease screening may not benefit from this study.

Inclusion criteria:

People with conditions for which clinical guidelines recommend annual screening for kidney disease:

* Diabetes
* Hypertension
* People with a chronic kidney disease diagnosis.
* People with a rapidly declining eGFR (loss of 15% in the last 3 months or \> 5 ml/min/1.73m2 per year).
* People with albuminuria (\>3 mg/mmol)
