What this trial studies

ACHIEVE P1-EPI is the prospective observational component of the ACHIEVE GREATER Center focused on patients with above-normal blood pressure seen at Wayne Health Mobile Health Unit events in Detroit. It combines a retrospective analysis of MHU patients beginning in April 2020 with a prospective sub-cohort who consent to ongoing follow-up through electronic health records and, for a subset, home blood pressure monitoring. Enrolled participants must be 18 or older, have elevated blood pressure (≥120 systolic or ≥80 diastolic), and be able to receive text messages; the protocol includes text-message reminders as part of its intervention tools. The study will analyze clinical data, health care utilization, and social determinants of health to identify drivers of poor blood pressure control and inform targeted prevention strategies.

Who should consider this trial

Good fit: Adults (18+) who attended Wayne Health Mobile Unit events in Detroit with systolic ≥120 or diastolic ≥80, who have a phone that can receive text messages, and who consent to ongoing follow-up and EHR review.

Not a fit: People with normal blood pressure (<120/<80), pregnant individuals, children under 18, those without a text-capable phone, those unable to consent, or those enrolled in another mobile-unit interventional trial are unlikely to benefit from this project.

Inclusion Criteria:

* Mobile Health Unit Patients with blood pressure above normal (≥120 systolic and/or ≥80 mmHg)
* Has a phone with the ability to receive text messages
* 18+ years old
* Consent to allow prospective follow-up including through EHR review

Exclusion Criteria:

* Non-mobile health unit patients
* MHU patients with systolic BP \< 120 mmHg AND diastolic BP \< 80 mmHg
* Pregnant Women
* Children less than 18 years old
* Individuals viewed by the investigative team as unable to understand and sign the informed consent form
* Currently enrolled in another on-going interventional trial initiated on the mobile health unit
