What this trial studies
This study aims to enhance the uptake of produce prescription programs (PRx) among Hispanic Medicaid-eligible patients with Type 2 Diabetes in Connecticut by implementing a community health worker (CHW) strategy. Initially, a small pilot will test the CHW approach with 10 participants, followed by a randomized controlled trial comparing outcomes between participants receiving CHW support and those who do not. The study focuses on overcoming barriers to PRx uptake and improving health outcomes through personalized support from CHWs. The ultimate goal is to evaluate the effectiveness of this user-centered implementation strategy.
Who should consider this trial
Good fit: Ideal candidates for this study are Hispanic individuals with Type 2 Diabetes who are eligible for Medicaid and reside in Hartford County.
Not a fit: Patients who do not speak English or Spanish, have gestational diabetes, or belong to vulnerable populations such as incarcerated individuals may not benefit from this study.

Eligibility criteria

Inclusion Criteria: * Be a current primary or specialty care patient at Hartford Hospital * Live in Hartford County * Identify as Hispanic * Have a current diagnosis of Type II Diabetes * Be eligible for Medicaid based on household income and size. Exclusion Criteria: * Does not speak English or Spanish * Has a diagnosis of gestational diabetes * Individuals who are members of vulnerable populations, such as individuals who are incarcerated and individuals who are not able to consent. Participants who are pregnant will be eligible for the study if they meet all of the inclusion criteria and do not have any of the exclusion criteria, as the study activities (additional funds to purchase fruits and vegetables, nutrition education, and support from a Community Health Worker) does not pose any additional risk to this group.
