This study will see if taking several blood pressure medicines before elective heart surgery causes bigger drops in blood pressure after anesthesia and increases the need for emergency blood-pressure drugs.

What this trial studies

This is a prospective observational study of adults aged 40–85 with ASA III–IV undergoing elective cardiac surgery at a single center. Investigators will record patients' chronic antihypertensive medications and group them by drug classes and combinations. Hemodynamics will be monitored with invasive arterial pressure during induction, and post-induction hypotension (PIH) burden will be quantified as the area under the curve for MAP <65 mmHg. The total dose of vasoactive drugs (for example, ephedrine) given to maintain pressure will also be recorded to compare effects of different medication combinations.

Who should consider this trial

Good fit: Adults aged 40 years or older scheduled for elective cardiac surgery who have been taking at least one antihypertensive medication regularly for four weeks or more are the ideal candidates.

Not a fit: Patients not on chronic antihypertensive therapy, those needing emergency surgery, or patients with severe organ failure or very low ejection fraction are unlikely to benefit from the study's findings.

Inclusion Criteria:

* Adult patients aged 40 years and older
* Undergoing elective cardiac surgery (e.g., isolated CABG, isolated valve surgery, or combined CABG+valve surgery via sternotomy)
* Regular use of at least one antihypertensive medication for at least 4 weeks prior to the operation date
* Voluntary participation and signed informed consent

Exclusion Criteria:

* Emergency surgery
* Preoperative shock or requirement for high-dose inotropic/vasopressor therapy
* End-stage liver or kidney failure
* Uncontrolled hypertension
* Difficult intubation or prolonged induction process
* Ejection fraction less than 35%
