This study will see if insomnia makes children and teens with obstructive sleep apnea less likely to stick with positive airway pressure (PAP) therapy.

What this trial studies
This multicenter observational cohort will follow 82 children aged 8–18 with obstructive sleep apnea who are newly prescribed PAP therapy at two Canadian pediatric hospitals. Participants will complete insomnia and sleep questionnaires and have objective PAP usage data collected over time to measure adherence. The study will analyze whether insomnia symptoms predict lower PAP use and whether race or ethnicity influence adherence patterns. The longitudinal design is intended to clarify temporal relationships between insomnia and PAP adherence in pediatric patients.
Who should consider this trial
Good fit: Ideal candidates are children 8–18 with obstructive sleep apnea (AHI ≥ 5/hour) who are newly prescribed PAP therapy and can complete questionnaires.
Not a fit: Children under 8, those already using PAP, those with developmental disabilities that prevent answering questionnaires, or those with primarily non-obstructive sleep disorders may not receive benefit from this study.
Why it matters
Potential benefit: If successful, the study could identify insomnia as a treatable factor to improve PAP use and sleep outcomes for children with OSA.
How similar studies have performed: Adult studies have shown that treating insomnia can improve PAP adherence, but longitudinal data in pediatric populations are novel and unproven.

Inclusion Criteria: 1. Children aged 8-18 years old 2. Obstructive apnea-hypopnea index ≥ 5/hour on baseline diagnostic polysomnogram (PSG) 3. Newly prescribed PAP therapy Exclusion Criteria: 1\) Developmental disability precluding ability to answer questionnaires
