How different types of low back pain affect pain and quality of life after failed lumbar surgery
Effects of Low Back Pain Types on Pain Severity and Quality of Life in Patients With Failed Back Surgery Syndrome
This project will test whether neuropathic, nociceptive, or nociplastic low back pain are linked to pain severity and quality of life in adults with failed back surgery syndrome.

What this trial studies
This prospective, hospital-based observational project will enroll 200 adults with failed lumbar spine surgery syndrome who have had low back pain for at least three months and present to the outpatient physical therapy and rehabilitation clinic at Konya Beyhekim Training and Research Hospital. Participants will have a single assessment using the Visual Analog Scale (VAS) and Pain Catastrophizing Scale (PCS) for pain, the Roland-Morris Disability Questionnaire (RMDQ) for function, and the SF-36 for quality of life. Pain type will be classified using the DN4 for neuropathic pain and clinical criteria/grading for nociplastic pain, with remaining cases labeled nociceptive. Outcomes will compare pain intensity, disability, catastrophizing, and quality-of-life scores across the different pain-type groups.
Who should consider this trial
Good fit: Adults aged 18 or older with a diagnosis of failed back surgery syndrome and at least three months of low back pain who can attend a single outpatient visit at Konya Beyhekim Training and Research Hospital.
Not a fit: People under 18, those with major psychiatric illness or communication barriers, or patients whose back pain has other identified causes would likely not benefit from participating in this project.
Why it matters
Potential benefit: If successful, results could help clinicians match treatments to pain mechanisms and improve symptom control and quality of life for patients with failed back surgery syndrome.
How similar studies have performed: Previous observational research has shown that distinguishing neuropathic, nociceptive, and nociplastic pain can predict differences in pain severity and quality of life, but there is limited published data specifically in failed back surgery syndrome.
