What this trial studies

This observational study will perform detailed metabolic profiling of malignant cells, immune cells, and extracellular metabolites from bone marrow samples across disease stages and treatment time points. Participants include adults with hematologic malignancies (AML, MDS, ALL, primary or post‑ET/PV myelofibrosis) and healthy donors, with samples analyzed for cellular respiration, metabolite levels, and immune cell function. The goal is to map metabolic interactions that may drive therapy resistance and relapse, particularly in the context of allogeneic hematopoietic stem cell transplantation. Good fit: Adults (over 18) who can give informed consent and have a diagnosis of AML, MDS, ALL, primary myelofibrosis, or post‑ET/PV myelofibrosis are the intended participants.

Not a fit: Pregnant or lactating people, those unable to consent, patients with other active malignancies within the past three years, and healthy donors should not expect direct therapeutic benefit from participation.

Inclusion Criteria:

* age \> 18 years,
* signed written informed consent
* For participants with hematological cancers: diagnosis of MDS, AML, ALL, PMF or post-ET/PV MF

Exclusion Criteria:

* pregnant or lactating women,
* inability to give consent,
* active malignant disease (other than MDS, AML, ALL, PMF or post-ET/PV MF in the last 3 - years prior to study inclusion).
* For healthy donors: previous or current hematological disease, previous or current other malignant disease,
