We will try giving nicotine replacement therapy and behavioural support to hospital patients who smoke and their household members to see if both quit smoking more often and stay quit longer.

What this trial studies
This project invites adult inpatients at Royal Papworth Hospital who smoke, plus any household members who also smoke, to join the Treating Tobacco Dependency (TTD) program together. Participants receive nicotine replacement therapy and structured behavioural counselling delivered initially by the TTD team and followed by the NHS-approved provider HealthyYou. Outcomes (quit rates and relapse) are measured at 28 days, 3 months, and 12 months after the quit date. Results will be compared against historical TTD program data from before household members were included.
Who should consider this trial
Good fit: Adults (18+) admitted to Royal Papworth Hospital who smoke, live with other smokers, are eligible for the TTD programme, and live in or are registered with a GP in the Cambridgeshire & Peterborough area.
Not a fit: People who do not live with other smokers, are pregnant, have contraindications to NRT, have household members unwilling to participate, or are already enrolled in another cessation programme may not receive benefit from this household-focused approach.
Why it matters
Potential benefit: If successful, this approach could increase quit rates and reduce relapse for both patients and their household members.
How similar studies have performed: Some prior household- or partner-based cessation programs have shown improved quit rates in certain settings, but overall evidence has been mixed.
Eligibility criteria

Inclusion Criteria: 1. Any inpatient aged 18 and above admitted to Papworth Hospital who smoke. (Smoking is described as using any tobacco products as defined by the TTD programme) 2. Patient and household members must be eligible to join the TTD programme. 3. Patients must live in a household with other smokers. 4. Participants must live in Cambridgeshire and/or be registered with a GP Practice in the Cambridgeshire \& Peterborough Integrated Care System Exclusion Criteria: 1. Pregnant individuals - Pregnant individuals who smoke are offered a differing pathway to the standard TTD programme. 2. Contraindications for NRT. 3. Any household member who smokes unwilling to participate. 4. Individuals already enrolled on smoking cessation programme.
