What this trial studies

This project aims to assess the feasibility of managing hypertension through home blood pressure monitoring combined with regular consultations with pharmacists and physicians. The study will enroll 24 participants from the Emergency Department Post-Discharge Clinic at UAB, dividing them into two groups: one that measures blood pressure at home and receives weekly pharmacist support, and another that continues with standard care. Participants will be provided with a blood pressure device and will complete questionnaires regarding their health and satisfaction with the program over a 3-month period.

Who should consider this trial

Good fit: Ideal candidates are adults aged 18 and older who have hypertension and are English speakers with access to a smartphone.

Not a fit: Patients with severely elevated blood pressure (≥160 mmHg) may not benefit from this study.

Inclusion Criteria:

* Patients seen at the Emergency Department Post-Discharge Clinic
* \>=18 years
* Blood pressure \>=130/80 mmHg and \<=160/100 mmHg
* English speakers
* Have a smartphone

Exclusion Criteria:

* Patients with blood pressure \>=160 mmHg
