This project will try a home-based program of exercise, nutrition, and psychological support to see if it is feasible and acceptable for adults waiting for elective coronary artery bypass graft surgery.

What this trial studies
CABGpreHAB is a randomized pilot at Rigshospitalet that compares a home-based multimodal prehabilitation program plus usual care versus usual care alone for adults scheduled for elective first-time CABG. The intervention combines tailored exercise, nutritional advice, and psychological support delivered in the weeks before surgery, with remote and in-person monitoring of adherence and fidelity. Primary outcomes are feasibility measures such as recruitment, retention, attrition, fidelity, and adherence, while clinical and functional measures are collected to inform a larger trial. Eligible participants are adults who speak Danish, are at least two weeks from surgery, and have no unstable cardiac conditions or contraindications to exercise.
Who should consider this trial
Good fit: Ideal candidates are Danish-speaking adults (≥18 years) scheduled for elective isolated first-time CABG with at least two weeks before surgery who can safely perform exercise and provide informed consent.
Not a fit: Patients with recent unstable angina or MI, significant left main stenosis, LVEF <35%, severe heart failure (NYHA IV), major arrhythmias, cognitive or orthopedic limitations, or inability to consent are unlikely to benefit or be eligible.
Why it matters
Potential benefit: If successful, the program could increase patients' functional capacity before surgery, reduce postoperative complications, and speed return to independence.
How similar studies have performed: Multimodal prehabilitation has shown benefits in some elective non-cardiac surgeries, but randomized data specifically for CABG are limited, so this approach is relatively novel in cardiac surgery.

Inclusion Criteria: * Planned to have elective isolated first-time CABG surgery. * 18 years or older * Two weeks or more before surgery * Speaks and understands Danish. * Gives written informed consent. Exclusion Criteria: * Patient with unstable angina pectoris and/ or MI during the last two weeks. * Left main stem stenosis \>50% * Left ventricular ejection fraction \< 35% * Significant ventricular arrhythmia * Orthopedic or neurologic preconditions precluding exercise training. * Inability to attend the prehabilitation program due to physical limitations. * Patients who have cognitive deficits that would disqualify prehabilitation. * Severe heart failure, NYHA Class IV * Lack of capacity to consent
