What this trial studies
This clinical trial aims to understand the strengths and challenges faced by older adults who have recently re-entered their communities after incarceration. It will develop a tailored psychoeducational intervention to enhance their chronic disease management. Participants will complete surveys and engage in focus groups to inform the intervention, followed by an 8-week program designed to improve their health outcomes. The study will assess the intervention's acceptability and effectiveness among this population.
Who should consider this trial
Good fit: Ideal candidates are individuals aged 45 and older who have re-entered the community from incarceration and have at least one chronic health condition.
Not a fit: Patients who are younger than 45, have not been incarcerated, or do not have a chronic health condition may not benefit from this study.

Inclusion Criteria: * at least 45 years old * have re-entered the community from a period of incarceration from jail or prison * have at least one chronic health condition * able to speak and understand English Exclusion Criteria: * aged 44 or younger * have not re-entered the community from a period of incarceration from jail or prison * does not have at least one chronic health condition * unable to speak and understand English
