This project will see how often HFpEF appears in people aged 50–70 hospitalized with COPD and whether having HFpEF changes their symptoms, exercise capacity, and prognosis.

What this trial studies
This observational study compares HFpEF detection rates in hospitalized COPD patients and examines how the presence of HFpEF affects clinical course and outcomes. Investigators will perform a retrospective chart review to identify previously undiagnosed HFpEF and compare recorded diagnoses with echocardiographic and hemodynamic criteria. Routine blood tests and biomarker analysis, echocardiography (including epicardial adipose tissue thickness and E/e'), spirometry, body plethysmography, and a 6‑minute walk test will be used to characterize participants. The study is conducted at sites in Kazan and Moscow and enrolls Russian citizens aged 50–70 hospitalized for COPD.
Who should consider this trial
Good fit: Ideal candidates are Russian citizens aged 50–70 with a verified diagnosis of COPD who are hospitalized at a participating center and can give informed consent.
Not a fit: People without COPD, those outside the 50–70 age range, individuals with other decompensated chronic diseases, or those unable to consent (including non‑Russian citizens) are unlikely to benefit from this study.
Why it matters
Potential benefit: If successful, earlier recognition of HFpEF in people with COPD could allow more targeted treatment and potentially improve symptoms and long‑term outcomes.
How similar studies have performed: Previous observational studies have documented overlap between COPD and HFpEF and shown that HFpEF is associated with worse functional status and prognosis, so this work builds on established findings rather than testing an entirely novel concept.

Inclusion Criteria: 1. Verified diagnosis of COPD; 2. Patients hospitalized with COPD; 3. Citizenship of the Russian Federation; 4. Patients aged 50 to 70 years; 5. Patient consent to participate in the study. Exclusion Criteria: 1. Lack of Russian Federaion citizenship; 2. Patient's refusal to participate in the study; 3. Any chronic diseases, other than COPD, that affect the prognosis, in the acute and/or decompensated stage; 4. Mental disorders.
