What this trial studies
This trial evaluates a heart failure risk assessment and prevention strategy for patients with type 2 diabetes using clinical risk scores and cardiac biomarkers. Primary care providers will receive notifications through electronic health records when their diabetic patients are identified as high risk for heart failure. The study aims to implement evidence-based preventive therapies and facilitate expert consultations or referrals to specialized programs. The goal is to enhance patient outcomes through proactive management of heart failure risks.
Who should consider this trial
Good fit: Ideal candidates for this study are primary care providers who manage patients with type 2 diabetes.
Not a fit: Patients who do not have diabetes or those whose primary care providers do not participate in the study will not benefit.
Why it matters
Potential benefit: If successful, this approach could significantly reduce the incidence of heart failure in patients with type 2 diabetes.
How similar studies have performed: Other studies have shown promise in using similar risk assessment and prevention strategies, indicating potential for success in this approach.
