Guiding PEEP adjustments in patients with pulmonary fibrosis on mechanical ventilation

End-expiratory Trans-pulmonary Pressure Guided PEEP Titration in Patients With Pulmonary Fibrosis and UIP Pattern Undergoing Mechanical Ventilation

Observational University of Modena and Reggio Emilia · NCT05098717

What this trial studies

This observational study focuses on patients with pulmonary fibrosis and usual interstitial pneumonia who require mechanical ventilation due to acute respiratory failure. It aims to document the effects of adjusting positive end-expiratory pressure (PEEP) based on trans-pulmonary pressure measurements. By optimizing lung mechanics and oxygenation, the study seeks to improve clinical outcomes for these patients, who often face poor prognoses. The approach is novel as it specifically targets the unique challenges posed by fibrotic lungs during mechanical ventilation.

Who should consider this trial

Good fit: Ideal candidates include individuals diagnosed with pulmonary fibrosis exhibiting a UIP pattern who are undergoing controlled mechanical ventilation for acute respiratory failure.

Not a fit: Patients with chest wall deformities or chronic obstructive pulmonary disease may not benefit from this study.

Why it matters

Potential benefit: If successful, this study could lead to improved ventilation strategies that enhance oxygenation and reduce complications in patients with pulmonary fibrosis.

How similar studies have performed: While similar approaches have been explored in patients with acute respiratory distress syndrome, the specific application to pulmonary fibrosis has not been extensively documented, making this a novel investigation.
