Gadolinium-weighted MRI to better find brain metastases
A Prospective Comparative Study of a Novel Gadolinium-Weighted MRI Sequence Versus Conventional T1-Weighted Imaging for the Detection of Brain Metastases
This will test a new MRI technique after gadolinium contrast to see if it finds more or smaller brain metastases in adults having a routine contrast brain MRI.

What this trial studies
This prospective comparison enrolls adults with known or suspected brain metastases who are already scheduled for a clinical gadolinium-enhanced brain MRI. Each participant receives the routine contrast-enhanced MRI followed by an additional gadolinium-weighted sequence without increasing the contrast dose or overall scan time. Images from the novel sequence will be compared to standard MRI images to determine whether it improves detection of lesions. The observational study is conducted at Karolinska University Hospital in Solna, Sweden.
Who should consider this trial
Good fit: Adults (18+) scheduled for a clinical gadolinium-enhanced brain MRI with known or suspected brain metastases who can provide consent and lie still for the scan.
Not a fit: Patients with MRI or gadolinium contraindications, severe renal impairment (eGFR < 30 mL/min/1.73 m²), pregnant or breastfeeding individuals, or those unable to comply with procedures are unlikely to benefit.

Inclusion Criteria: * Age 18 years or older * Scheduled for clinical brain MRI with gadolinium contrast * Known or suspected brain metastases * Able to provide informed consent * Able to lie still for the duration of the MRI examination Exclusion Criteria: * Contraindications to MRI * Known allergy or contraindication to gadolinium-based contrast agents * Renal impairment (eGFR \< 30 mL/min/1.73m²) * Pregnant or breastfeeding * Inability to comply with study procedures
