What this trial studies

This study aims to understand and improve long-term viral suppression among people living with HIV (PLWH) who are not optimally engaged in care. Utilizing the REMAIN App, researchers will follow a diverse cohort of PLWH for up to 48 months, focusing on those at high risk for or currently experiencing viral non-suppression. The study will collect data through online surveys, app-based check-ins, and remote biomarkers to evaluate care engagement and viral load patterns. By addressing syndemic factors such as substance use, mental health, and barriers to care, the study seeks to enhance the effectiveness of HIV treatment strategies.

Who should consider this trial

Good fit: Ideal candidates include individuals over 18 years old living with HIV who are not currently virally suppressed or engaged in care, and who may have active substance use or mental health disorders.

Not a fit: Patients who are virally suppressed and not at risk of becoming unsuppressed in the near future will not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* Have access to a personal smartphone device;
* Over 18 years of age;
* Speak English or Spanish;
* Living with HIV
* Are either 1) not currently virally suppressed, 2) not currently engaged in care, or 3) have one or more factors associated with viral non suppression including a history of missed appointments or viral non suppression;
* Active substance use or mental health disorder;
* Willing and able to provide written informed consent to take part in the study.

Exclusion Criteria:

* Unwilling or unable to provide reliable contact information;
* Unwilling to provide blood sample for HIV virology testing;
* Living with HIV and virally suppressed and not at risk of becoming unsuppressed in the near future.
