What this trial studies
This study aims to address the increased needs for care and support among elderly individuals living at home, particularly in the wake of the COVID-19 pandemic. It utilizes the Évaluation et orientation SOcio-GÉRiatrique (ESOGER) questionnaire, which is available on a secure digital platform, to rapidly assess the health and social needs of vulnerable elderly populations. The goal is to identify those most in need and provide tailored recommendations for care and services. By focusing on the most vulnerable, the study seeks to improve access to necessary support for elderly individuals.
Who should consider this trial
Good fit: Ideal candidates for this study are elderly individuals aged 65 and older who live at home in the specified territory and exhibit mild to severe frailty.
Not a fit: Patients who do not reside within the designated territory or who are unable to understand the study materials in French or English may not benefit from this study.
Why it matters
Potential benefit: If successful, this intervention could significantly enhance the quality of life for elderly individuals by ensuring they receive appropriate care and support.
How similar studies have performed: While this approach is tailored to the current context of increased vulnerability due to the pandemic, similar interventions targeting elderly care have shown promise in improving health outcomes.
