What this trial studies

This implementation study assesses a differentiated service delivery model aimed at improving care for people living with HIV (PLWH) who also have chronic comorbidities. Utilizing a Hybrid Type 3 study design, the research focuses on evaluating implementation outcomes and client service outcomes through the RE-AIM framework. Participants will be recruited from an HIV clinic in Hong Kong, where they will complete a survey to express their willingness to participate. The study aims to address the challenges of managing comorbidities in PLWH by exploring innovative healthcare delivery methods.

Who should consider this trial

Good fit: Ideal candidates for this study are adults aged 18 and older living with HIV who have one or more cardiometabolic comorbidities and are receiving care at the HIV clinic.

Not a fit: Patients who are unable to provide informed consent or do not have any comorbidities may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* PLWH eligible to participate in this study are PLWH aged 18 years or above
* followed up at the HIV Clinic, and with one or more cardiometabolic comorbidities, including hypertension, diabetes, hyperlipidaemia, metabolic-associated fatty liver disease, cardiovascular diseases, and cerebrovascular diseases. They will be eligible if HIV physicians consider them having sub-optimal control of their comorbidities, suspected of having complications, or would benefit from screening of complications resulting from their comorbidities. These indications are selected as they involve specialist care, including investigations and treatment, which are not available at the HIV Clinic.

Exclusion Criteria:

* Incompetent subjects who cannot give informed written consent.
