What this trial studies

This study aims to evaluate how different medical treatments prior to in vitro fertilization (IVF) affect fertility outcomes in women with endometriosis-related infertility. Participants will be randomly assigned to one of three groups: one receiving combined oral contraceptive pills, another receiving Dienogest, and a control group proceeding directly to IVF/ICSI. The study will monitor ovarian response through hormone levels and ultrasound examinations during the stimulation phase, aiming to optimize the number of mature follicles for egg retrieval. The ultimate goal is to improve fertilization rates and overall success in achieving pregnancy.

Eligibility criteria

Inclusion Criteria:

* Presence of Endometrioma on ultrasound.
* History of ovarian cystectomy either open or via laparoscopic surgery.
* Presence of endometriotic foci in the pelvis during laparoscopic surgery.
* Patients coming for the first ICSI / IVF cycle.

Exclusion Criteria:

* Age above 40 years
* Cases with endometrioma on ultrasound more than 3 cm size.
* Prior failed ICSI / IVF.
* Known poor responders or poor ovarian reserve, defined as AFC\<5 or AMH \< 0.5 .
* Patients with previous uterine surgery.
* Patients diagnosed to have uterine abnormalities
