Digital self- and remote monitoring for chronic pain in primary care to improve quality of life

This study will test whether adults with chronic pain can use the Paindrainer app and CARE portal to better monitor their symptoms and activity and improve quality of life while managed in primary care.

What this trial studies
This feasibility study implements Paindrainer®, a CE-marked Class I system that combines a patient smartphone app and a CARE portal for healthcare providers. Participants will use the app to self-monitor activity levels and symptoms while clinicians use the CARE portal for remote review and feedback. The system applies advanced neural networks to deliver personalized feedback aimed at helping patients plan daily activities and track their impact on pain. The study will track implementation feasibility, user engagement, and changes in quality of life and care workflows in primary care.
Who should consider this trial
Good fit: Adults over 18 with pain affecting daily life for more than three months who speak Swedish or English, have access to and are willing to use a smartphone or tablet, have no planned surgeries during the study, and are motivated to try digital self-monitoring.
Not a fit: People with severe or acute mental illness, active substance use disorder, serious illnesses under active treatment, cancer-related pain, ongoing litigation related to back/neck pain, or no access/ability to use a smartphone are unlikely to benefit from this intervention.
Why it matters
Potential benefit: If successful, the tool could help patients better self-manage chronic pain, improve daily functioning, and enable primary care teams to allocate specialist resources more efficiently.
How similar studies have performed: Previous digital self-monitoring and remote care programs for chronic pain have shown modest improvements in self-management and outcomes, but the use of neural-network personalized feedback like Paindrainer remains relatively novel and under-studied.

Inclusion Criteria: Has pain that affects daily life and has lasted for more than 3 months Is over 18 years old Understands Swedish or English Is motivated and willing to use a digital tool for self-monitoring of activity level and symptoms Has no planned surgeries during the study period Has knowledge of and access to using a smartphone or tablet Exclusion Criteria: Severe or acute mental illness, severe anxiety, or depression Ongoing history of substance use disorder Serious illness under active treatment Pain related to malignancy Currently involved in legal proceedings or ongoing litigation related to low back or neck pain \-
