What this trial studies
This program aims to empower individuals with Chronic Obstructive Pulmonary Disease (COPD) through a digital platform that facilitates guided self-determination (GSD) follow-ups by primary care nurses. The intervention focuses on enhancing self-management skills and improving the quality of life for patients living with COPD. By integrating digital home-based care within a general practice setting, the study explores the feasibility of this approach in primary healthcare. The goal is to address the challenges faced by COPD patients and contribute to a sustainable healthcare system.
Who should consider this trial
Good fit: Ideal candidates are individuals diagnosed with COPD who are living at home and meet specific pulmonary function criteria.
Not a fit: Patients with severe somatic diseases, severe psychiatric diagnoses, or those unable to understand Norwegian may not benefit from this program.
Why it matters
