This project will try ASSIP and ACT alongside usual care to see which better lowers suicidal thoughts and mental pain in adults who recently attempted suicide.

What this trial studies

This randomized controlled trial will enroll 60 adults who made a suicide attempt in the past four months and randomly assign them to ASSIP plus usual care, ACT plus usual care, or usual care alone. Interventions are delivered in an outpatient health center in Eslamshahr and outcomes are measured at pre-test, post-test, and follow-up. Primary outcomes include suicidal ideation measured by the Beck Scale for Suicide Ideation (BSSI) and mental pain measured by the Orbach & Mikulincer Mental Pain Scale (OMMP). The trial excludes people with severe illnesses that interfere with treatment, current substance abuse, or those receiving other structured psychological therapies.

Who should consider this trial

Good fit: Adults aged 18 or older who can read and write, can give informed consent, and have had at least one suicide attempt within the past four months are ideal candidates if they are not receiving other structured psychological interventions and do not have severe interfering illness or substance abuse.

Not a fit: People with severe psychiatric or physical illnesses that interfere with treatment, current substance abuse, minors under 18, or those already engaged in structured psychotherapy may not receive benefit from participation.

Why it matters

Potential benefit: If successful, one of the therapies could more effectively reduce suicidal ideation and mental pain and help prevent repeat suicide attempts among recent attempters.

How similar studies have performed: Both ASSIP and ACT have shown promising results internationally for reducing suicidal ideation and reattempts, but direct head-to-head comparisons are limited and evidence in the Iranian context is sparse.

Inclusion Criteria:

* Being 18 years of age or older
* Having the ability to read and write
* Providing informed consent to participate in the study
* Having at least one suicide attempt in the past 4 months

Exclusion Criteria:

* Having a severe psychiatric or physical illness that interferes with the treatment process
* Receiving concurrent structured psychological interventions (other than TAU) that may interfere with the study's treatment process.
* Lack of informed consent to participate in the study
* Substance abuse based on DSM-5 criteria
