What this trial studies

This study evaluates the effectiveness of Community Health Worker (CHW)-led interventions aimed at addressing social risks that impact the management of uncontrolled diabetes and hypertension among patients in community health centers. The mixed-methods approach includes screening for social risks and providing referrals to appropriate services. The study aims to measure improvements in blood sugar levels and blood pressure among participants, while also identifying effective strategies for expanding CHW-led programs. Findings will guide community health centers in implementing programs that address social determinants of health.

Who should consider this trial

Good fit: Ideal candidates for this study are adults aged 18 and older with uncontrolled Type 2 diabetes or hypertension.

Not a fit: Patients who are under 18 years old, pregnant, or do not meet the criteria for Type 2 diabetes or hypertension may not benefit from this study.

Why it matters

Potential benefit: If successful, this intervention could significantly improve health outcomes for patients with uncontrolled diabetes and hypertension by addressing underlying social barriers.

How similar studies have performed: Other studies have shown promise in using community health workers to address social determinants of health, suggesting potential for success in this approach.
