What this trial studies
This study focuses on developing a culturally tailored community-based prevention model to reduce the risk of colorectal cancer (CRC) and cardiovascular disease (CVD) among African Americans. The intervention includes a Screening, Brief Intervention, and Referral to Treatment (SBIRT) approach for CRC screening, alongside a web-based lifestyle program called 'Alive!' to address CVD risk factors. The study will compare the effectiveness of this community health worker-led intervention against usual care and evaluate its impact on dietary habits and overall health metrics. The goal is to improve screening rates and health outcomes in a population that faces significant disparities in CRC incidence and mortality.
Who should consider this trial
Good fit: Ideal candidates for this study are English-speaking Black individuals aged 45 years and older who are not up-to-date with colorectal cancer screenings.
Not a fit: Patients who are non-English speaking or younger than 45 years may not benefit from this study.

Inclusion Criteria: 1. English-speaking 2. Aged 45 years and older 3. Not up-to-date with CRC 4. Working telephone 5. Can provide informed consent Exclusion Criteria: 1. Non-English speaking 2. \<45 years old
