What this trial studies
This intervention focuses on addressing high rates of maternal mortality and adverse pregnancy outcomes among Black postpartum patients, particularly those experiencing preeclampsia. The study will adapt an existing community health worker program to provide tailored support and education on cardiovascular disease prevention. Participants will be randomized to receive either standard care or enhanced support from community health workers. The pilot aims to assess the feasibility and acceptability of this intervention in improving health outcomes for this underserved population.
Who should consider this trial
Good fit: Ideal candidates are Black postpartum patients aged 16-56 who have experienced adverse pregnancy outcomes and plan to deliver at UAB Hospital.
Not a fit: Patients who do not identify as Black, are not experiencing adverse pregnancy outcomes, or are currently incarcerated may not benefit from this study.

Inclusion Criteria: * Self Identifies as Black * Between 16-56 years old * Experiencing adverse pregnancy outcomes defined as hypertensive disorders of pregnancy, preterm birth, placental abruption, pregnancy loss (loss at greater or equal to 14 weeks gestation), gestational diabetes, delivering a small for gestational age infant * Planning to deliver at UAB Hospital * Speaks and writes in English Exclusion Criteria: * Declines Randomization * Speaks or writes in languages other than English * Currently incarcerated * Previously enrolled in P3OPPY
