This pilot compares antibiotic resistance in bacteria that cause uncomplicated UTIs among people who seek care at community drug outlets versus hospitals in Kampala, Uganda.

What this trial studies
This is an observational pilot that collects urine and carriage samples from symptomatic patients presenting with uncomplicated UTIs at participating community drug retail outlets (CDROs) and partnering hospital/clinic sites in Kampala. Isolates will undergo culture and antimicrobial susceptibility testing, and resistance profiles will be compared between CDRO-derived and hospital-derived samples and against the WHO-GLASS database. The study will also document patient care pathways, antibiotic use behaviors, and the appropriateness of antimicrobial therapy. A secondary comparison will examine differences between two neighbourhoods in Kampala to explore local variation.
Who should consider this trial
Good fit: People of any age in Kampala who have acute symptoms of an uncomplicated UTI (for example recent dysuria, urgency, frequency, suprapubic pain, or acute worsening of pre-existing urinary symptoms in older adults) and who present to participating CDROs or the collaborating clinic/hospital are ideal candidates.
Not a fit: People without UTI symptoms and those with complicated, long-standing, or primarily hospital-acquired infections are unlikely to benefit from this community-focused pilot.
Why it matters
Potential benefit: If successful, findings could help guide better empirical antibiotic choices for UTIs and strengthen local antibiotic stewardship in community settings.
How similar studies have performed: Global surveillance programs such as WHO-GLASS have documented uropathogen resistance, but direct comparisons of resistance between community drug outlets and hospital-derived isolates in Kampala are limited, making this pilot relatively novel.

Inclusion criteria: * Must have one or more of the following clinical presentations regardless of age: acute (\< 2 weeks) dysuria. increased urinary urgency and frequency, irritation, discharge. increased lower abdominal pain or discomfort and sometimes gross haematuria. * In elderly patients with pre-existing urinary symptoms: increased acute urinary changes. Exclusion criteria: * People without symptoms of UTI.
