This project will try to find what helps or gets in the way of colorectal cancer screening for patients aged 45–54 at community health centers by talking with patients and staff and linking staff survey results to clinic records.

What this trial studies
This observational, mixed-methods project uses the CFIR implementation framework to identify system-level determinants of colorectal cancer screening in unscreened patients aged 45–54. The team will conduct a validated organizational survey with staff and providers at Codman Square Health Center and nine partner community health centers and link those survey responses to patient-level electronic health record data. Researchers will also do one-time in-depth interviews with patient-facing community health center staff, providers, and patients, with interviews conducted online or in person. Findings will be used to map barriers and facilitators within clinic systems that influence screening completion.
Who should consider this trial
Good fit: Ideal candidates are patients aged 45–54 who receive care at the participating community health centers, have not completed colorectal cancer screening, and can give verbal or written consent.
Not a fit: People with a personal or family history of colorectal cancer, inflammatory bowel disease, high-risk cancer syndromes, or those who have already completed screening are excluded and are unlikely to benefit from this observational work.
Why it matters
Potential benefit: If successful, the findings could help clinics change processes to increase screening rates and detect colorectal cancer earlier in people aged 45–54.
How similar studies have performed: Previous implementation and CFIR-based studies have identified clinic-level barriers to screening and supported targeted improvements in some settings, so the approach has precedent.

PATIENTS Inclusion Criteria: * Individuals aged 45-54 * Individuals who are patients at the health center * Individuals that are willing and able to give verbal/written consent Exclusion Criteria: * Individuals with a personal history of CRC * Individuals with a family history of CRC * Individuals with a history of inflammatory bowel disease or high-risk cancer syndrome STAFF Inclusion Criteria: * Individuals ages 18+ * Individuals working at the health center * Individuals that are involved in CRC screening processes at CSHC * Individuals that are willing and able to give verbal/written consent
