This project will test patient- and system-level strategies to increase follow-up colonoscopy after a positive FIT for patients aged 45–75 at a Boston community health center.

What this trial studies
The project uses mixed methods guided by the Consolidated Framework for Implementation Research (CFIR 2.0) to identify patient- and system-level determinants of follow-up colonoscopy after a positive FIT. Investigators will develop equity-driven implementation strategies matched to those determinants and pilot them at Codman Square Health Center. Pilot outcomes and feasibility will be measured using the RE-AIM framework with an explicit equity lens. The setting focuses on a predominantly Black, low-income clinic population with historically low screening and follow-up rates.
Who should consider this trial
Good fit: Ideal candidates are patients aged 45–75 who receive care at Codman Square Health Center and are willing to provide consent, especially those with a recent positive FIT or who are due for colorectal cancer screening.
Not a fit: Patients with a personal or family history of colorectal cancer, inflammatory bowel disease, high-risk cancer syndromes, or those not receiving care at Codman Square or unwilling to consent are unlikely to benefit from participation.
Why it matters
Potential benefit: If successful, these strategies could increase timely diagnostic colonoscopy after positive FIT and help reduce colorectal cancer disparities in this high-risk community.
How similar studies have performed: Previous implementation efforts and patient-navigation interventions have improved screening and follow-up in some settings, but follow-up after positive FIT remains variable and often insufficient, so results have been mixed.

Inclusion Criteria: * Individuals aged 45-75 * Individuals who are patients at Codman Square Health Center * Individuals that are willing and able to give verbal/written consent Exclusion Criteria: * Individuals with a personal history of colorectal cancer * Individuals with a family history of colorectal cancer * Individuals with a history of inflammatory bowel disease or high-risk cancer syndrome
