This work will test whether placing clips on the polyp removal site prevents bleeding in adults who restart anticoagulant medications (excluding aspirin) after colon polyp removal.

What this trial studies
This is a retrospective, observational analysis using a large single-center registry of more than 10,000 colonoscopies to compare adults who received preventive hemostatic clips after polypectomy with those who did not. The population of interest is patients who restarted anticoagulant therapy after the procedure (acetylsalicylic acid excluded). Researchers will compare rates of post-polypectomy colorectal bleeding after anticoagulant restart and exclude cases where closure was performed with a suturing device or full-thickness resection device. The goal is to determine whether routine preventive clipping is associated with lower delayed bleeding in this anticoagulated population.
Who should consider this trial
Good fit: Adults who underwent colorectal polypectomy and subsequently restarted systemic anticoagulant therapy (other than aspirin) are the intended population for this analysis.
Not a fit: Patients whose mucosal defects were closed with a suturing device or full-thickness resection device, patients who never restarted anticoagulants, or those only on aspirin would not be expected to benefit from the conclusions of this analysis.
Why it matters
Potential benefit: If effective, prophylactic clipping could lower delayed bleeding rates after polypectomy in patients who resume anticoagulants, reducing complications and need for additional interventions.
How similar studies have performed: Prior studies and guidelines report mixed results—selective clipping and clipping large right-sided EMR defects can reduce bleeding, but evidence is inconsistent and there is no clear recommendation specific to patients on anticoagulants.

Inclusion Criteria: * adult who has received a colonoscopy with endoscopic colorectal polypectomy * patient who has received at least one anticoagulant after polypectomy Exclusion Criteria: * Endoscopic polypectomy was combined with closure of the mucosal defect by a suturing device or full thickness resection device
