What this trial studies

This mixed-methods pilot adapts the evidence-based CAPABLE home-based program to patients enrolled in Johns Hopkins Home-Based Medicine (JHOME) to address social isolation and disability. Participants receive an individually tailored 4-month intervention delivered by an occupational therapist, a registered nurse, and a handy worker with a series of home visits and supportive services. Eligibility focuses on JHOME patients aged 50 or older who report social isolation or loneliness and are cognitively intact; outcomes include function in ADLs/IADLs and measures of social connection. The study integrates quantitative measures and qualitative feedback to test feasibility and potential impacts before scaling.

Who should consider this trial

Good fit: Ideal candidates are JHOME enrollees aged 50 or older who live in Baltimore City or County, have some difficulty with ADLs, screen positive for isolation or loneliness, are cognitively intact (MOCA > 23), and are not receiving active cancer treatment.

Not a fit: Patients who are terminally ill, live in long-term care settings, have significant cognitive impairment, have frequent recent hospitalizations, or live outside Baltimore City/County are unlikely to benefit or be eligible.

Potential benefit: If successful, this combined approach could reduce loneliness and improve independence in daily activities, raising quality of life for homebound older adults.

How similar studies have performed: CAPABLE itself has shown improvements in ADLs, IADLs, and depression, but combining CAPABLE with home-based primary care specifically to address social isolation is a novel application with limited prior evidence.

Eligibility criteria

Inclusion Criteria:

* Any patient currently in JHOME
* At risk for or experiencing social isolation as defined by the Lubben Social Network Scale 6 item score less than or equal 12 points or loneliness as defined by the UCLA Loneliness Scale 3 item score of 6 to 9 points
* Cognitive inclusion criteria Montreal Cognitive Assessment (MOCA) \>23 as determined by referring JHOME study team member provider
* English speaking (measures are standardized in English)
* Ability to participate in an approximately 45-60 min virtual or in person meeting
* Eligible for CAPABLE
* 50 years or older
* Not hospitalized over night more than 4 times in the last 12 months
* Have some difficulty with any ADL
* Cognitively intact
* Live in Baltimore County or Baltimore City
* Not receiving active cancer treatment
* Interested in participating in CAPABLE Care partners will be included if the care partners provide \> 10 hours of care/week

Exclusion Criteria:

* Terminally ill
* live in long term care setting
* receiving active cancer treatment

Standard of Care/Comparison group Inclusion Criteria:

• Any patient currently in JHOME who did not receive the CAPABLE intervention
