What this trial studies
This randomized controlled trial will enroll 750 adults with moderate to severe opioid use disorder who are new mobile health MOUD patients at Prisma Health Upstate in South Carolina. Participants will be randomized 1:1 to receive either the mobile health clinic (MHC) standard protocol or MHC plus a Certified Peer Support Specialist (PSS) delivering Assertive Community Engagement services. The trial will follow participants for six months and measure primary outcomes of MOUD initiation and retention, with secondary analyses including cost-effectiveness and optimization of mobile clinic–PSS allocation. The intervention links patients visiting mobile clinics to peers in recovery who provide targeted recovery support to improve engagement and reduce overdose risk.
Who should consider this trial
Good fit: Adults aged 18 or older with a diagnosis of opioid use disorder who are new patients seeking MOUD at Prisma Health mobile clinics in South Carolina and can provide informed consent are ideal candidates.
Not a fit: People already working with a certified peer support specialist, those with severe medical or psychiatric conditions that prevent participation, or those unable to read and understand consent materials are unlikely to benefit or be eligible.

Inclusion Criteria: * Age 18+ * Have diagnosis of OUD * New mobile health MOUD patient Exclusion Criteria: * Unable to read and comprehend the consent materials and other study materials * Currently working with a Certified Peer Support Specialist * Having severe medical or psychiatric disability that would hinder participation in the study, as determined by the clinical provider
