This project will enroll people with Alzheimer's who test positive for AD biomarkers to see how biomarkers and other risk factors relate to disease progression in older adults in China.

What this trial studies

This prospective, single-center cohort will recruit patients meeting NIA-AA criteria for AD-related mild cognitive impairment or probable AD who are amyloid-positive by PET or cerebrospinal fluid testing. Participants will be followed and characterized with clinical assessments and biomarker data to explore links between biomarkers, clinical features, and progression. Investigators will analyze potential risk factors for faster decline and use those findings to build a biomarker-based early warning model of progression rate. The cohort is intended to create a resource for future AD research and to enable earlier, more precise prevention strategies for patients in China.

Who should consider this trial

Good fit: Adults aged 40 or older with AD-related mild cognitive impairment or probable AD who are amyloid-positive by PET or CSF testing and can provide informed consent at Xuanwu Hospital.

Not a fit: People who are amyloid-negative, unable to complete assessments because of severe psychiatric or physical disorders, or unable to attend the Beijing center are unlikely to benefit from this cohort.

Why it matters

Potential benefit: If successful, the project could enable earlier identification of people at higher risk of rapid decline, allowing more targeted monitoring and preventive care.

How similar studies have performed: Large international biomarker cohorts such as ADNI have successfully linked biomarkers to clinical progression, so this effort applies a validated approach to a Chinese population.

Eligibility criteria

Inclusion Criteria:

* (1) Patients aged 40 years or older;
* (2) Visiting Xuanwu Hospital, Capital Medical University;
* (3) Meeting the core clinical diagnostic criteria for Alzheimer's disease (AD) - related mild cognitive impairment or probable AD according to the NIA-AA (2011);
* (4) Amyloid positivity confirmed by amyloid PET or cerebrospinal fluid biomarker tests;
* (5) Providing informed consent.

Exclusion Criteria:

* (1) Individuals who are unable to complete the assessment due to severe psychological or physiological disorders.
