What this trial studies
This pilot randomized trial will enroll 40 patients with ARDS and randomize them to daily upright bed positioning or to standard care with head-of-bed elevation. The intervention consists of using an upright bed position each day while patients remain hospitalized, compared with routine head-of-bed elevation. Key exclusions include active cancer and the presence of an active tracheostomy. The trial is conducted as a single-center feasibility study at the University of Alabama at Birmingham.
Who should consider this trial
Good fit: Hospitalized adults with a clinical diagnosis of ARDS who do not have active cancer or an active tracheostomy would be eligible.
Not a fit: Patients with active cancer, an active tracheostomy, or other contraindications to changing bed position may not receive benefit from this approach.
Why it matters
Potential benefit: If successful, the intervention could improve oxygenation and comfort and reduce the need for more invasive measures such as proning.
How similar studies have performed: While prone positioning has demonstrated benefit in severe ARDS, daily upright bed positioning is less studied and represents a relatively novel, pilot-level approach.
