What this trial studies

This Phase 3 interventional trial compares two regional nerve block techniques—anterior iliopsoas space block and PENG block—used before spinal anesthesia for hip surgeries. Participants will receive one of the two blocks using bupivacaine to provide perioperative analgesia and to facilitate patient positioning for spinal anesthesia. Investigators will measure ease of positioning as the primary outcome and will also collect data on postoperative pain, opioid consumption, and early mobility. The trial enrolls adult patients (ASA I–II) undergoing procedures such as dynamic hip screw fixation or hemiarthroplasty at Benha Faculty of Medicine in Egypt.

Who should consider this trial

Good fit: Adults over 18 with recent hip fracture scheduled for hip surgery under spinal anesthesia (ASA class I–II and surgery within about two weeks) are the ideal candidates.

Not a fit: Patients with contraindications to spinal anesthesia or peripheral nerve blocks, those with ischemic heart disease, chronic opioid use, or significant cognitive impairment are unlikely to benefit or be eligible.

Why it matters

Potential benefit: If successful, the better block could make spinal anesthesia positioning easier, reduce pain, and lower opioid needs after hip surgery.

How similar studies have performed: PENG block has shown benefit in prior clinical reports for hip analgesia and positioning, while the anterior iliopsoas space block is a newer technique with limited clinical outcome data.

Eligibility criteria

Inclusion Criteria:

* (ASA )classes I and II
* patients of either sex, above the age of 18 who will undergo hip surgeries( (Dynamic hip screw fixation or hemiarthroplasty)(not older than 2 weeks) with persistent pain and scheduled for surgery under SA with an expected duration of 2.5 hours

Exclusion Criteria:

* patient's refusal to participate
* any contraindications to SA or peripheral nerve blocks
* history of ischemic heart disease
* patients on opioids for chronic pain
* patients with significant cognitive impairment.
