What this trial studies
This clinical trial evaluates the effectiveness of a fall prevention program delivered by Advanced Practice Nurses (APNs) compared to standard care for non-frail elderly patients aged 80 and older. Participants will undergo a comprehensive geriatric assessment and may voluntarily engage in a home-based physical activity program over 12 months. The study will track fall incidents at multiple intervals to assess the impact of the intervention. Conducted in multiple primary care centers in the Pays de la Loire region of France, the trial aims to provide insights into the role of APNs in enhancing elderly care.

Eligibility criteria

Inclusion criteria: * Aged 80 or over; * Vigorous according to the " Clinical Frailty Scale " (scores 1, 2, 3) ; * Having a referring primary care physician working in pairs with the local Advanced Practice Nurse; * affiliated to a Health Care Plan * Informed and having signed consent to participate in the research. Exclusion criteria: * Under guardianship or trusteeship; * Unable to travel to the primary care office or unable to follow the adapted physical activity program; * Poor understanding of French.
