Zadek Osteotomy for Treatment of Haglund's Syndrome
This project will try Zadek osteotomy surgery to relieve heel pain and improve function in adults aged 18–60 with Haglund's syndrome who have not improved after at least six months of conservative care.

What this trial studies
This is a prospective case series conducted at the Orthopedics and Traumatology department of Sohag University hospitals in Sohag, Egypt. Adults with symptomatic Haglund's syndrome refractory to six months of conservative treatment will undergo a dorsal closing-wedge calcaneal osteotomy (Zadek osteotomy) and be followed for short-term clinical, radiological, and functional outcomes. Patients with Achilles tendon rupture/degeneration, uncontrolled diabetes, active infection, Charcot joint, or peripheral vascular disease will be excluded. Outcomes will include pain, footwear comfort, function, and imaging measures of calcaneal anatomy.
Who should consider this trial
Good fit: Adults aged 18–60 with symptomatic Haglund's syndrome who have failed at least six months of conservative treatment and do not have tendon rupture, severe systemic disease, active infection, Charcot joint, or peripheral vascular disease are the intended candidates.
Not a fit: Patients with Achilles tendon rupture or degeneration, uncontrolled diabetes, active osteomyelitis or other infections, Charcot joint, peripheral vascular disease, or those outside the 18–60 age range are unlikely to benefit from this procedure in this protocol.
Why it matters
Potential benefit: If successful, the procedure could reduce posterior heel pain, improve footwear comfort and activity, and restore function for patients with refractory Haglund's syndrome.
How similar studies have performed: Zadek osteotomy and similar calcaneal osteotomies have been reported in case series with favorable short-term outcomes, but high-quality comparative trials are limited.

Inclusion Criteria: * patients over 18 years of age and less than 60 Yrs * All patients had at least six months of conservative treatment Exclusion Criteria: 1. patients below 18 years of age and more than 60 Yrs 2. patients of Haglund's syndrome with AT rupture or degeneration 3. uncontrolled diabetic patients with or without neuropathic joint destruction, 4. Infection such as osteomyelitis. 5. Charcot joint. 6. Peripheral vascular disease
