This project will screen adults in Xinqiao who are at high risk for diabetes or have prediabetes and use continuous glucose monitoring and health checks to track blood sugar patterns, eye and bone health, and see who progresses to diabetes.

What this trial studies

The project will create a standardized cohort of adults in the Xinqiao/Songjiang area identified as high-risk for diabetes or with prediabetes through community screening. Participants will use continuous glucose monitoring (CGM) and undergo ophthalmology and bone density exams, with clinical and lifestyle data collected at baseline and during follow-up. Investigators will track annual conversion rates from prediabetes to diabetes, characterize early changes in glucose metabolism and pancreatic islet function, and document glycemic variability. The goal is to identify high-risk patterns and effective interventions and to develop a practical local management model for high-risk patients.

Who should consider this trial

Good fit: Adults (≥18) living in the Xinqiao/Songjiang area who have a smartphone and a family history of diabetes or a prior abnormal glucose test are ideal candidates.

Not a fit: People already diagnosed with diabetes, those with recent surgery, acute cardiovascular events, active infections, other conditions judged unsafe for participation, or without a smartphone or ability to attend hospital visits are unlikely to benefit.

Why it matters

Potential benefit: If successful, this project could identify people most likely to develop diabetes early and enable targeted actions to prevent or delay disease onset.

How similar studies have performed: Previous cohort and CGM-based research has shown that monitoring glucose patterns can reveal early dysglycemia and predict progression, but applying these methods as a local community management model is relatively novel.

Inclusion Criteria:

* Individuals who provide written informed consen
* Have access to a smartphone
* Hamily history of diabetes or abnormal microglucose level detected once
* Age ≥18 years at the time of consent

Exclusion Criteria:

* Diabetes mellitus
* Any other condition which, in the judgment of the investigator, would make the subject unsuitable for enrollment in the study
* recently surgical histories, trauma, acute cardiovascular complications, or infectious diseases
