What this trial studies
This is a retrospective analysis of gastric biopsy cases that underwent H. pylori immunohistochemical staining at the First Hospital of Jilin University from March 1, 2023 to August 15, 2025. Two experienced pathologists independently reviewed IHC results and cases were classified using the Kimura-Takemoto system into non-atrophic versus atrophic and non-open versus open atrophy groups. Cases were also categorized as early gastric cancer or non-cancer based on established endoscopic submucosal dissection (ESD) criteria, and H. pylori positivity was graded by colonization extent. The study compares H. pylori distribution across these subgroups and uses multivariate analysis to identify factors associated with early gastric cancer.
Who should consider this trial
Good fit: Adults aged 18–80 who had positive H. pylori immunohistochemical testing at the First Hospital of Jilin University with adequate biopsies from both the greater and lesser curvatures of the antrum and body.
Not a fit: Patients with prior stomach surgery, suspected autoimmune gastritis, liver cirrhosis, severe comorbidities (such as advanced cancer or renal failure), advanced gastric cancer or gastric lymphoma, or inadequate biopsy sampling are unlikely to be included or benefit from these findings.
Why it matters
Potential benefit: If successful, the results could help clinicians and pathologists target and time early gastric cancer screening by showing typical H. pylori distribution patterns in precancerous and early cancer stages.
How similar studies have performed: Prior studies have linked H. pylori location and burden to atrophic changes and cancer risk, but detailed IHC-based mapping across defined atrophy stages and strict ESD-based early cancer criteria is relatively uncommon and partially novel.

Inclusion Criteria: * age 18-80 years * received H. pylori immunohistochemical testing at our center and tested positive. Exclusion Criteria: * biopsies not collected from both the greater and lesser curvatures of the gastric antrum and body * history of stomach surgery * suspected autoimmune gastritis under endoscopy * liver cirrhosis * severe comorbidities such as advanced cancer or renal failure * advanced gastric cancer or gastric lymphoma
