What this trial studies

The Aging Brain Care Virtual Program is an intervention designed to support patients with Alzheimer's disease and related dementias through virtual visits conducted by registered nurses or social workers. Over the course of one year, care partners will receive tailored monthly visits for the first three months, followed by quarterly check-ins based on individual needs. The program utilizes validated assessment tools to identify and address the specific needs of patients and caregivers, focusing on managing behavioral symptoms, caregiver stress, medication, comorbidities, and advance care planning.

Good fit: Ideal candidates for this program are individuals aged 65 and older with a diagnosis of Alzheimer's disease or related dementias who are active primary care patients.

Not a fit: Patients who are enrolled in hospice care or living in long-term care facilities may not benefit from this program.

Eligibility criteria

Show full inclusion / exclusion criteria
Patient Inclusion Criteria:

* 65 and older,
* active primary care patient (visit in past 14 months),
* diagnosis of Alzheimer's disease and related dementias (ADRD) based on ICD criteria in primary care problems list.

Patient Exclusion Criteria:

* enrolled in hospice at the study start (study screening)or
* living in a long term care nursing facility.

Care Partners Inclusion Criteria:

* 18 and older,
* able to access a phone or computer for intervention delivery in English, and
* identified in the EMR as the emergency contact or as the primary care partner.

Care Partners Exclusion Criteria:

- Individuals who decline to participate in the ABCV intervention
