This study will try two different neck glide techniques to see which one eases cervicogenic headache in adults aged 18 to 44.

What this trial studies

This randomized controlled trial assigns adults with cervicogenic headache by coin toss to receive either ventro‑cephalic or dorso‑caudal translatoric glides. Diagnosis uses ICHD‑3 clinical criteria with clinical or imaging evidence of a cervical source. Eligible participants are 18–44 years old and excluded if they have migraine or tension‑type headache, vertigo/dizziness, prior head/neck surgery, recent trauma, or are already receiving manual therapy. Treatments and outcome assessments are performed at Foundation University College of Physical Therapy in Islamabad.

Who should consider this trial

Good fit: Adults aged 18–44 diagnosed with cervicogenic headache by ICHD‑3 clinical criteria and with clinical or imaging evidence of a cervical source are ideal candidates.

Not a fit: People with migraine or tension‑type headache, a history of vertigo or dizziness, prior head/neck surgery, recent trauma, or those already receiving manual therapy are unlikely to benefit from this comparison.

Why it matters

Potential benefit: If successful, the preferred glide technique could reduce headache pain and improve neck function for people with cervicogenic headache.

How similar studies have performed: Prior trials of cervical mobilization and manipulation have shown modest benefits for cervicogenic headache, so this comparison builds on a partly supported approach.

Eligibility criteria

Inclusion Criteria:

* • Age 18 to 44 years old

  * Both male and female A- Any headache fulling criterion C. B- Clinical and/or imaging evidence of lesion or disorder in cervical spine or soft tissues of neck that can cause headache.

C- Evidence of cause of headache demonstrated by at least 2 of these:

1. headache has developed in temporal relation to the onset of the cervical disorder or lesion.
2. headache has significantly improved or resolved in parallel with improvement in cervical lesion or disorder.
3. cervical range of motion is reduced, and headache is made significantly worse by provocative maneuvers.
4. headache is abolished following diagnostic blocked of cervical structure. D- Not better accounted for by another ICHD-3 diagnosis

Exclusion Criteria:

* 1- Tension-type headaches and migraine 2- History of Vertigo or Dizziness 3- Already taking manual therapy treatment 4- Prior surgery to the head and neck 5- Trauma
