What this trial studies
This study aims to evaluate the effectiveness of video-based Diabetes Self-Management Education and Support (DSMES) and a combined approach with community health worker (CHW) support for improving glycemic control in Chinese immigrants with uncontrolled Type 2 diabetes in New York City. Participants will be randomly assigned to one of three groups: one receiving weekly DSMES videos, another receiving the videos plus bi-weekly support calls from a CHW, and a control group continuing their usual care. The CHW will also address social determinants of health barriers and connect participants to community resources. The study will last for 24 weeks, with the goal of enhancing diabetes management and health outcomes.
Who should consider this trial
Good fit: Ideal candidates are Chinese immigrants aged 18-70 with a diagnosis of Type 2 diabetes and an HbA1c of at least 8%.
Not a fit: Patients who are unable to participate meaningfully in the intervention due to impairments or those who are pregnant or breastfeeding may not benefit from this study.

Eligibility criteria

Inclusion Criteria: * self-identify as Chinese American; * be above 18 years old; * have a diagnosis of Type 2 diabetes (T2D) in the medical record; * have had an appointment with a physician for routine T2D care within the past 12 months; * have a most recent HbA1c of at least 7%; * be willing to receive brief videos regarding T2D management; and * possess a smartphone or, if they do not have one, be willing and able to use a study smartphone. Exclusion Criteria: * unable or unwilling to provide informed consent; * unable to participate meaningfully in the intervention (e.g., uncorrected sight and hearing impairment); * unwilling to accept randomization assignment; * is pregnant, plans to become pregnant in the next 6 months, or becomes pregnant during the study; or * is breastfeeding (e.g., they may have potential dietary restrictions).
