What this trial studies

This study evaluates the effectiveness of a digital program that combines personalized health coaching and remote patient monitoring using a connected blood pressure cuff. Who should consider this trial

Good fit: Ideal candidates are adults aged 18 and older with a clinical diagnosis of hypertension who are English-speaking and receive primary care at Stanford Primary Care.

Not a fit: Patients under 18 years of age, non-English speakers, or those currently enrolled in the Omada Program may not benefit from this study.

* Receives primary care at Stanford Primary Care

Exclusion Criteria:

* \<18 years of age
* Primary language not English
* Active or prior enrollment in Omada Program
