What this trial studies
This study evaluates the effectiveness of a digital program that combines personalized health coaching and remote patient monitoring using a connected blood pressure cuff. Participants will use a smartphone or website to track their blood pressure and receive tailored guidance to improve their hypertension management. The goal is to determine if this approach leads to better blood pressure control compared to standard care practices. The study targets individuals diagnosed with hypertension who receive primary care at Stanford.
Who should consider this trial
Good fit: Ideal candidates are adults aged 18 and older with a clinical diagnosis of hypertension who are English-speaking and receive primary care at Stanford Primary Care.
Not a fit: Patients under 18 years of age, non-English speakers, or those currently enrolled in the Omada Program may not benefit from this study.

Inclusion Criteria: * Clinical diagnosis of hypertension * 18+ years of age * English-speaking * Receives primary care at Stanford Primary Care Exclusion Criteria: * \<18 years of age * Primary language not English * Active or prior enrollment in Omada Program
