What this trial studies
This study investigates the effects of the late sodium current blocker mexiletine on patients diagnosed with giant T-wave electrical alternans, a condition that can lead to serious heart arrhythmias. By targeting the late sodium current, the study aims to reduce the risk of ventricular tachycardia and ventricular fibrillation, which are precursors to sudden cardiac death. Patients will be randomized to receive either mexiletine or conventional therapy to assess the efficacy of this intervention in preventing life-threatening arrhythmias.
Who should consider this trial
Good fit: Ideal candidates for this study are patients clinically diagnosed with macroscopic T-wave alternans.
Not a fit: Patients with acute coronary syndrome, progressive myocardial ischemia, or those contraindicated for mexiletine will not benefit from this study.

Inclusion Criteria: * Clinical diagnosis of macroscopic TWA Exclusion Criteria: * The patients who do not agree to participate in the study, * Patients with acute coronary syndrome or with progressive myocardial ischemia according to clinical manifestations, electrocardiogram or myocardial biochemical markers; * Those who have used other class I antiarrhythmic drugs or who are contraindicated by mexiletine.
